  • Seeking a physician fellowship trained in pain medicine, PM&R, board certified in anesthesia.
  • The qualified individual would be providing pain care with an emphasis on all advanced interventional modalities treating chronic pain.
  • Coverage includes private practice and ambulatory surgery sites in addition to hospital locations. Our practice currently consists of 13 physicians and 4 cnps with planned additional growth. Our facilities have state of the art equipment and physical therapy.
  • This practice is located in and around Toledo, OH which offers cultural diversity, excellent schools and neighborhoods along with a centrally located large international university.
  • No nights or weekends

Compensation Details

  • Employment Agreement with three-year initial term with automatic renewal. Standard language regarding performance, termination with or without cause, non-competition and/or lost profit amount determination
  • Compensation structure flexible

Option 1: 100% productivity model –   Employee receives 100% of net profit after practice and personally identified expenses (same compensation model as Partners)

  • $22,000 monthly advance on earnings, then adjusted monthly draws based on productivity to maximum $35,000 per month. Quarterly productivity bonus. Flexible PTO

Option 2: Guaranteed base salary model “A”

  • $340,000 base salary year one, 85% productivity bonus
  • $380,000 base salary year two provided qualified for bonus in year one, 85% productivity bonus

Option 3:  Guaranteed base salary model “B”

  • $380,000 base salary year one, 55% productivity bonus
  • $420,000 base salary year two with 85% productivity bonus providing qualified for bonus in year one
  • Year three and subsequent years compensation based on productivity model with 85% productivity bonus above monthly draw of $35,000. 

Standard benefit package consistent with other Physician Employees or Partners including but not limited to: 

  • Medical malpractice insurance

  • Annual CME stipend: $4,000 per year subject to Manager approval
  • Participation in 401(k) retirement plan beginning on the date of employment
  • Twenty (20) days Paid Time Off (PTO). Additional time off may be granted by the Medical Director. Additional PTO available in any subsequent term year providing productive benchmarks are met.

Partnership Track:

  • Partnership opportunity may be offered anytime, though generally requires Board Certification in your primary specialty along with certification in Pain Medicine.
  • Partnership provides highest income potential including 100% of net productivity along with more flexible PTO and potential for equity investment in certain related entity business ventures (such investment is not required). 
  • Ownership interest in ASC available at fair market value six months after employment commencement date providing Employee meets Stark Safe Harbor requirements. Partnership not required for ASC ownership.
